Texas True Choice (TTC) is pleased to offer the employee benefit plan participants of Ft. Bend County, Texas an on-line directory of the providers available throughout Texas. This on-line directory is for your reference and includes all providers that are available to you in Texas. As an additional convenience to our members, your search results will be displayed in a printable directory customized to your specifications.

The Ft. Bend County Employee Benefit Plan has selected the Texas True Choice (TTC) physician and ancillary provider network throughout Texas (which also includes the University of Texas Medical Branch – Galveston (UTMB) Hospital and Physicians, Doctors Hospital Parkway, Doctors Hospital Tidwell, Bayside Community Hospital and Sweeny Community Hospital). All other participating hospitals throughout Texas have been contracted through the Private Healthcare Systems, (PHCS) network. All participating providers in Texas available to Ft. Bend County, Texas employee benefit plan participants are included in this special section of the TTC website for your convenience.

PLEASE NOTE: Ft. Bend County, Texas requires that ALL inpatient hospital admissions be pre-certified. In addition, all employee benefit plan participants should contact a representative with the Ft. Bend County, Texas E.A.P. program prior to contacting a PPO participating Mental Health or Chemical Dependency provider for care. Please refer to your benefit I.D. card, and your employee benefit plan booklet for additional information.

The networks make every effort to maintain current and accurate provider data; however, periodic changes to a provider's status can occur. Please contact the Customer Service telephone number on your benefit I.D. card prior to receiving treatment to confirm the provider's participation in the networks.

Depending on your health plan, the provider you select may not be available due to plan restrictions. In addition, using a participating PPO network provider does NOT guarantee that all services will be covered under your health plan. To verify your benefit eligibility information, contact your plan administrator at the number listed on your benefit I.D. card, or consult your employer's Risk Management Department representative.

The PHCS hospitals located in this provider search are contracted through PHCS, not TTC. TTC is listing the PHCS hospitals on our website as a convenience to Ft. Bend County, Texas. The participating providers listed in this directory are independent contractors and are not employees of the networks. All health care decisions are strictly between the patient and the provider. The networks do not participate in the patient-provider relationship.
