Montgomery County Provider Search -- (Hospitals outside of Texas)

Texas True Choice (TTC) is pleased to offer the employee benefit plan participants of Montgomery County, Texas a listing of the hospital providers available outside of Texas. This special section of the TTC website includes only those providers. Montgomery County, Texas has contracted with Private Healthcare Systems (PHCS) for all hospitals located outside of Texas. TTC is listing the PHCS hospitals on our website as a convenience to Montgomery County, Texas.

PLEASE NOTE: Montgomery County, Texas requires that ALL inpatient hospital admissions and outpatient surgical procedures be pre-certified. In addition, ALL employee benefit plan participants are required to contact a representative with the Montgomery County, Texas E.A.P. program PRIOR to contacting a PPO participating Mental Health or Chemical Dependency provider for care. Please refer to your benefit I.D. card and your employee benefit plan booklet for additional information.

Every effort is made to maintain current and accurate provider data; however, periodic changes to a provider's status can occur. Please contact the Customer Service telephone number on your benefit I.D. card prior to receiving treatment to confirm the provider's participation in the networks.

Depending on your health plan, the provider you select may not be available due to plan restrictions. In addition, using a participating PPO network provider does NOT guarantee that all services will be covered under your health plan. To verify your benefit eligibility information, contact your plan administrator at the number listed on your benefit I.D. card, or consult your employer's Risk Management Department representative.

The participating providers listed in this directory are independent contractors and are not employees of the networks. All health care decisions are strictly between the patient and the provider. The networks do not participate in the patient-provider relationship.
